2008 Application Instructions

To be considered all applications must be accompanied by:

1. Three letters of recommendation to include:

a. High School Teacher or Counselor
b. Community Leader
c. Ministry Leader other than the CMBC Youth Pastor

2. A photograph of yourself (a headshot not larger than 2”x3”)

3. An official transcript from high school or institution of higher learning

4. Proof of your ACT or SAT Test Scores

5. An essay, not to exceed 200 words, which includes information about yourself, your ministry involvement, and your educational goals. Please explain how this award will help you to achieve these goals. Tell why you should receive this award.

6. Prior Year Income Tax Statement (Required only if you wish to be considered for either of the following scholarships: E.K. Bailey or Page-Reeves Scholarships)

7. Please list the year(s) that you have previously applied for scholarship funds through the Concord Scholarship Program.

8. Please provide name(s) and date(s) of all Concord Scholarship(s) received.

RETURN THE APPLICATION PACKAGE TO THE ADDRESS INDICATED ON THE APPLICATION. PLEASE SUBMIT APPLICATION PACKET IN THE FOLLOWING ORDER:

1. Completed application
2. Transcript
3. Letters of recommendation
4. Essay
5. Tax information (if applicable)
6. Photo of yourself – please write your name on the back
Please attach a letter of explanation for any items to be sent under separate cover (i.e. transcripts); however, please note that these items must arrive by the deadline date and that it is the responsibility of the applicant to make sure that all the documents arrive by the deadline. Applicants will not be notified regarding missing or late items.

Please use the following information to determine your eligibility and to choose the scholarship application form which you should complete.

UNDERGRADUATE

  • Available to college-bound graduating high school seniors, Freshmen, Sophomores, Juniors, and Seniors planning to enter college in Fall 2007 semester.
  • Awarded for full-time study for one academic year.
  • Proof of enrollment/registration must be received. (Letters of admissions are not acceptable.)

GRADUATE

  • Available to graduate students working on a professional degree, masters, doctoral or enrolled in post-doctoral study.
  • Awarded for full-time study for one academic year

IMPORTANT DATES/MEETINGS:

Sat., June 28, 2008 - Notify Awardees
Award recipients (only) will be notified of the Scholarship Ministry’s actions.

Sat., July 12, 2008 - Scholarship Awards and Info. Brunch
Award recipients, or a representatives must attend.

Sun., July 20, 2008 - Worship Service
Award recipients must attend worship service.
